A few weeks ago, the American medical scene was an intriguing debate because there was an unconscious patient.
What makes the doctor argue is not about the condition of the patient, but because the patient has a tattoo on the chest that read "do not resuscitate" which means "Do not Resust / Wake / Turn"
The doctors were confused between saving the man or following the written message on his chest so as not to be helped.
What is Resustation?
Resuscitation (artificial respiration) is an attempt to provide adequate ventilation, adequate oxygen and cardiac output to supply oxygen to the brain, heart and other vital devices. (Maternal and Neonatal Health Service, 2002)
Resuscitation is breathing by applying heart massage and artificial respiration (Medical Dictionary, 2000 Edition).
Resuscitation is the act of reviving or restoring a person's apparent consciousness to die as a result of the cessation of heart and lung function, which is brain oriented (Tjokronegoro, 1998).
Meanwhile, according to Rilantono, et al (1999) resuscitation contains the literal meaning of "revive", which is meant efforts that can be done to prevent an episode of cardiac arrest continues to be biological death.
Cardiac pulmonary resuscitation consists of two main components: basic life support (BHD) and live support (BHL). Next is post-resuscitation treatment.
PURPOSE OF RESUSCITATION
Provide adequate ventilation
Limiting damage to serebral
Provision of oxygen and cardiac output is enough to channel oxygen to the brain, heart and other vital tools
To start or maintain an extra life uteri
SIGNS - SIGNS OF RESUSCITATION NEED TO BE DONE
Respiratory
If the respiratory assessment indicates that the baby is not breathing or that breathing is not strong.
See chest movement up and down, frequency and depth of breathing for 1 minute. Breathless breath means that breathing is ineffective and needs action, such as apneu.
If breathing has been effective ie in normal babies usually 30 - 50 x / min and crying, we move to the next assessment.
Heart rate - frequency
If the heart rate assessment shows that the baby's heart rate is irregular.
The heartbeat frequency should be> 100 per minute. The easiest and quickest way is to use a stethoscope or feel the umbilical cord.
Feeling arteria has the advantage of being able to monitor the frequency of heart rate continuously, calculated for 6 seconds (the result multiplied 10 = the frequency of heart rate for 1 minute) The assessment results;
When the frequency> 100x / minute and the baby breathe spontaneously, followed by assessing the skin color.
If the frequency <100x / min even if the baby breathes spontaneously becomes an indication for VTP (Positive Pressure Ventilation)
Skin color
If the skin color assessment indicates that the baby's skin color is pale or can be up to cyanosis
After breathing and heart frequency is good, the skin should be reddish. If there is still central cyanosis, oxygen is still given.
If there is cyanosis purifier, oxygen does not need to be given, because the blood circulation is still slow, partly because the temperature of the cold maternity room.
CONDITIONS REQUIRING RESUSCATION
Airway obstruction: due to mucus / blood / meconium, or due to the tongue that falls posteriorly.
Condition of respiratory depression due to drugs given to the mother eg anesthetic drugs, local analgesics, narcotics, diazepam, magnesium sulfate, etc.
Neurological damage.
Airway or cardiovascular abnormalities / damage or central nervous system, and / or congenital abnormalities that may cause respiratory / circulatory disorders.
Hypovolemic shock for example due to cord compression or bleeding
More important resuscitation is needed in the first few minutes of life. If too late, could adversely affect the quality of life of the next individual.
THINGS WHICH NEED TO BE ATTEMPTED IN RESUSCITATION
Skilled workers, good working team.
Understanding of basic physiology of breathing, cardiovascular, as well as progressive asphyxia processes.
Temperature / tool capability, ventilation, monitoring.
Drugs and fluids are needed.
